Adult Down Syndrome Center of Advocate Lutheran General Hospital.  A comprehensive medical resource providing multidisciplinary medical and psychosocial care for adults with Down syndrome, with an emphasis on health promotion, located in Park Ridge, Illionois.  Located on their website is information for adult health care.

Down Syndrome: Health Issues by Dr. Len Leshin.  An excellent web site on the pediatric medical issues in Down syndrome.  

The Down Syndrome Preventive Medical Check List was produced by the Down Syndrome Medical Interest Group.  It provides a checklist of additional tests and evaluations recommended specifically for children and adults with Down syndrome. The checklists cover Birth to Age 12 Years and 13 Years to Adulthood.  These checklists are to accompany the above DSMIG health care guidelines. 

The Down Syndrome Research Foundation produced a brochure in 2005 Changing Lives : Down Syndrome & the Health Care Professional that is an abbreviated format of the 1999 Health Care Guidelines listed above.

Growth charts for children with Down syndrome produced by Greg Richards includes two sections; Growth charts for children with Down syndrome and Head circumference charts for children with Down syndrome.

Health Records
The Canadian Down Syndrome Society has produced a Personal Health Record brochure designed for older teens and adults. The brochure is written for those with Down syndrome to keep track of their personal health information. The brochure includes a section for personal health information, a section for health care professionals and a section for personal stories. 

Family and physician management plan summary for children and adolescents with Down syndrome.  In: The Physician's guide to caring for children with disabilities and chronic conditions  / edited by Robert E. Nickel and Larry W. Desch -- Baltimore, MD: Paul H. Brookes, 2000.  This template can be used to track specific health care issues of children and adolescents with Down syndrome during each visit to the doctor. It is reproduced on the University of Illinois website with permission from the publisher.

Down Syndrome Medical Interest Group - UK : This site provides essential information for healthcare professionals on 'best practice' medical care for people with Down's syndrome in the UK and Ireland.  The DSMIG developed Surveillance Guidelines to set out a minimum standard of basic medical surveillance that is considered essential; cardiac diseasethyroid disorderhearing impairmentophthalmic problems and the appropriate monitoring of growth
